Parent Registration Checklist

Here are the 5 things that each player must do for registration with AFSC:

1)  Online Registration -  complete online

2)  Medical Release Form - print & fill out, get notarized, give to Team Manager

3)  Concussion Understanding Form - print & fill out, give to Team Manager

4)  Code of Conduct Form - print & fill out, give to Team Manager

5)  Water Bill (Required for all Phoenix Residents) - get a copy of a recent Water Bill and give to Team Manager

6)  New players only - Becky Salazar will need to verify your original birth certificate and she will need a copy of it.
