Donation Request Letter
This letter can be used by AFSC teams to solicit donations from companies or individuals.

This form can be filled out to provide receipts to donors seeking proof of a donation to a non-profit organization.

Incident form to be completed by parent, coach or team manager.  Should be sent to David Iannone, Club Vice President.

AFSC Drug Policy
The AFSC Drug Policy was adopted for the 2002-2003 season and beyond. A great deal of research and hard work was performed in formulating this final document, which was reviewed many times by AFSC coaches and board members as well as judicial and law enforcement officials. The AFSC Board of Directors feels that the final product is a complete and thorough set of guidelines for players and coaches of the AFSC.

Financial Aid Form - Updated for 2011/2012
